The Fellowship of Christian Sportsmen Youth Membership Application

This application is required to be completed for all youth who do not have a parent who is a Club members prior to participation in Club activities.  FCS youth membership is available to minors (under 18 years old) without regard to race, national origin, age, denominational preference, or political persuasion.  Please print and complete all blanks.  If not making an apparel purchase mail the completed application to Randy Rowley, FCS President, at 11612 Birchbark Trail, Austin, TX, 78750.  If making an apparel purchase mail the completed application and your check for the apparel to Kevin Wall, FCS Treasurer, at 205 Covala Drive, Cedar Park, TX, 78613.  If you are purchasing gear please make your check out to FCS.  Please do not send cash.  This application only has to be completed once (it does not have to be renewed).

Youth do not pay membership dues.  FCS Polo shirts, FCS T-shirts, FCS caps, FCS mouse pads, and FCS coffee cups, Dove Retreat Caps developed by former FCS President Paul Wilson, FCS Patches, and FCS Magellan button down shirts, are available for purchase and can be mailed to your address (shipping is $3.22 for a shirt and cap, $2.80 per shirt, $1.55 per cap, $.42 per patch, shipping is included in the price of the mouse pads, and shipping and insurance for coffee cups varies based on distance).  Please check the FCS Gear page for current inventories to prevent ordering more than we have on hand.

FCS "Button Down" Magellan Fishing Shirts ($19.09 each) ___ 3X-Large, ___ 2X-Large, ___ X-Large, ___ Large, ___ Medium.  These shirts are available by special order only.  These shirts will not be paid for from FCS funds, therefore they must be paid for in advance.  Make your check out to me (Randy Rowley) and mail it to 11612 Birchbark Trail, Austin, TX, 78750.  Short and Long sleeve are available.  The cost for short sleeved shirts (including screen printing) is $19.09 (shirt = $16.23 and screen printing = $2.86).  Long sleeved shirts cost $2.17 more.  3XL and larger sizes cost $2.17 more.  As an alternative you can bring me (Randy Rowley) a button down shirt that you already have and I will get it screen printed for you for $2.86 (the shirt must have a Velcro closure on the back - it cannot be sewn).  We do not keep an inventory of these shirts.  They will only be ordered with a regular shirt order of 24 or more T-shirts (we order shirts about twice a year).  Postage to mail these shirts is $2.74 each.
